Abstract

This article reflects on the concept of transition and its implications for education.

For an optimal transition between stages must have good coordination between professionals of different  institutions involved. This coordination between stages should be supported on 4 pillars: collaboration, attention diversity and coexistence, the curriculum and information. We propose a working model for doing such coordination more effective and therefore the transition between stages as smooth as possible. Finally, it discusses the role that the Educational Service Zone, Catalunya, should have in this process.
